Identifying Health Needs, Finding Solutions - Morton Plant Hospital
As a leading community-based health system in the greater Tampa Bay area, BayCare Health System is committed to meeting the wide-ranging health needs of our neighbors in every community we serve.
Responding to that commitment, BayCare conducted a comprehensive Community Health Needs Assessment (CHNA) to identify the most significant health needs across our region and develop strategies for addressing them.
We used a collaborative process in the communities surrounding each of our hospitals, listening to input from the people we serve and working with community organizations that share our commitment to improve health and quality of life.
We found that our region’s health concerns are pressing and diverse: from increasing access to affordable care to addressing chronic conditions like diabetes and asthma that are limiting quality of life for too many in our communities. BayCare, with more than 200 locations throughout the Tampa Bay area, already has many programs to address our region’s health needs. We are committed to doing even more – by collaborating with community partners on services and solutions, using the CHNA as our roadmap.

With our network of 11 not-for-profit hospitals, outpatient facilities, and services such as imaging, lab, behavioral health, and home health care, BayCare connects patients to a complete range of preventive, diagnostic, and treatment services for any health care need. Guided by the CHNA, we will continue to work toward improving the health of all we serve.
